I do not accept insurance plans as an in-network provider with the exception of United Healthcare for NYU School of Medicine Employees.

I will, however, provide you with a statement to submit to your insurance company for out-of-network re-imbursement. I accept payment by cash, check, or credit card.

Payment is required at the time of the appointment.
